Role of Cold Agglutinin Test in Diagnosis of Mycoplasma Pneumoniae Pneumonia in the Children

Abstract
Mycoplasma pneumoniae infections affect both the upper and lower respiratory tract and occur most frequently in school-aged children and young adults. The serum cold agglutinin test is a simple and useful procedure in mycoplasma pneumoniae
infection to
diagnose early in the amine the diagnostic value of cold agglutinin test in Mycoplasma pneumoniae pneumoni, we carried out clinical studies in 107 cases of M. pneumoniae pneumonia patients who had been admitted to the Department of Pediatrics,
Chung Ang
university Hospital from Jury 1989 to december 1991. M. pneumoniae pneumonia was confirmed by indirect hemaglutination test. Statistical anaysis by NcNemar's chi square statistic was done.
@ES The results were as follows:
@EN 1) Out of 407 pneumonia patients, 107 cases(26.3%) were confirmed as M. pneumoniae pneumonia
2) The mean age of the cases was 5.9¡¾2.8 years. the male to female ratio was 1.3 : 1.
3) M. pneumoniae pneumonia accounts for 47% & 33% of all pneumonias in childeren aged 6~7 years & 8~15 years, respectively.
4) Out of the 107 cases with M. pneumoniae pneumonia, the most thing of indirect hemagglutination titers and cold aggultinin teiters was 1 : 1280 and 1 : 64(51 cases, 47.7% & 25 cases, 23.1%).
5) out of the 300 cases of pneumonia caused by other than M. pneumoniae, the most thing among cold agglutinin titers was below 1 : 4(83 cases, 27.6%).
6) Cold agglutinin titer began to rise around 1 week from onset and reached the peak level around 2 weeks, indirect hemaglutination titer began to rise in 1 week from onset, reached the peak level in 3 weeks and diminished slowly thereafter.
7) We observed statistically signification between titers of cold agglutinin test and indirect hemagglutination test when compared using same sera(r=0.469, P<0.05).
8) In 107 cases of children with M. pneumoniae pneumonia, cold agglutinin titer was measured. When cold agglutinin titer was above 1 : 32, the sensitivity was 76.6% and specificity was 86.7%. the value of NcNemar's chi square statistic for
assessing
the significance of the difference between cold agglutinin test and indirect hemagglutination test was 3.01. therefore ther was not significant difference between these two proportions of the 0.05 level. We concluded that cold agglutinin in a
titer
of 1
: 32 or higher is strong suggestive the diagnosis of M. pneumoniae pneumonia.
